In accordance with Part 1 of Article 18 of Federal Law No. 38-FZ dated March 13, 2006 "On Advertising", by filling out this form, acting freely, by my own will and in my own interest, I hereby grant to RAYMED TRADING GROUP PVT Limited Liability Company (INN: 5029204750, OGRN: 1155029011169), registered at the address: 141001, Moscow Region, Mytishchi, 1st Vokzalnaya St., Bldg. 8 (hereinafter — RAYMED TG PVT LLC), my consent to receive advertising and informational materials from RAYMED TG PVT LLC regarding goods, services, events, promotions related to the products of RAYMED TG PVT LLC.

I provide this consent to receive advertising and informational materials via telecommunication networks through SMS messages, email messages, messengers, and telephone calls.

This Consent is effective indefinitely from the moment it is provided until it is revoked. I hereby confirm that any action of mine by clicking the "Send" button, placing a "✔" mark in the relevant boxes on the website https://raymed.ru/en constitutes a sufficient form of consent and allows the parties to confirm the fact of obtaining such consent. No additional evidence shall be required to further confirm my free will.

I am aware of the possibility of subsequently refusing to receive advertising and informational materials by sending a corresponding notice to RAYMED TG PVT LLC at the email address info@raymed.ru.
